VA Special Monthly Compensation (SMC): Levels K Through T, Aid and Attendance, and Housebound Rules
Special Monthly Compensation (SMC) is a tax-free VA benefit under 38 U.S.C. 1114(k)–(t) and 38 CFR 3.350 for service-connected loss or loss of use of organs or limbs, bilateral blindness or deafness, housebound status, or Aid and Attendance. SMC-K is paid in addition to basic schedular pay, while SMC-L through SMC-T provide higher tiers above 100%.

What is VA Special Monthly Compensation (SMC)?
Special Monthly Compensation (often searched as smc va) is a tax-free federal benefit within the veteran pay and benefits system. While standard schedular VA disability ratings from 10% to 100% under 38 U.S.C. 1114(a)–(j) measure average earning impairment, Congress enacted 38 U.S.C. 1114(k)–(t) and 38 CFR 3.350 and 38 CFR 3.352 to compensate veterans for service-connected loss or loss of use of organs or extremities, severe blindness or deafness, housebound status, or regular Aid and Attendance (A&A).
- Additive SMC Allowance (SMC-K): Under 38 U.S.C. 1114(k) and 38 CFR 3.350(a), SMC-K is a fixed monthly add-on paid in addition to basic schedular compensation (0% to 100%, where the 100% veteran-alone schedular rate is $3,938.58 per month effective 2025-12-01), or in addition to
SMC-SandSMC-LthroughSMC-N. - Replacement Higher Tiers (SMC-L through SMC-T): Under 38 U.S.C. 1114(l)–(t) and 38 CFR 3.350(b)–(j), levels L, M, N, O, P, R (R-1 and R-2), S, and T establish higher monthly tiers that replace the 100% schedular rate.
Consult the official VA Special Monthly Compensation Rates tables for current SMC rates, and compare standard 10%–100% schedular rates on our VA disability pay chart.

SMC-K explained: Loss or loss of use of organs and extremities (38 CFR 3.350(a))
Under 38 U.S.C. 1114(k) and 38 CFR 3.350(a), SMC-K pays a monthly add-on for each qualifying loss or loss of use—even at a 0% combined rating:
- One or More Creative Organs (38 CFR 3.350(a)(1)): Acquired absence or loss of use of one or both testicles (excluding undescended/congenitally undeveloped testicles), ovaries, uterus, or penis. Includes service-connected erectile dysfunction under Diagnostic Code 7522 (38 CFR 4.115b, Footnote 1), Female Sexual Arousal Disorder (DC 7632), testicular atrophy (diameter reduced to one-third of normal, or one-half or less with altered consistency, or biopsy-proven absence of spermatozoa), or atrophy from in-service mumps with orchitis.
- One Hand or One Foot (38 CFR 3.350(a)(2)): Amputation, or loss of use where no effective function remains (grasping/manipulation for a hand; balance/propulsion for a foot) beyond what a below-elbow or below-knee amputation stump with a suitable prosthesis provides. Includes extremely unfavorable complete knee ankylosis, complete ankylosis of two major joints of an extremity, lower-extremity shortening of 3½ inches or more, or complete paralysis of the external popliteal (common peroneal) nerve with footdrop and trophic/circulatory changes.
- Both Buttocks (38 CFR 3.350(a)(3)): Severe bilateral Muscle Group XVII (DC 5317) damage making it impossible, without assistance (own hands/arms or special appliance), to rise from a seated or stooped (fingers-to-toes) position and maintain postural stability.
- Blindness of One Eye (38 CFR 3.350(a)(4)): Anatomical loss of one eye, or light perception only—inability to recognize test letters at 1 foot and perceive objects, hand movements, or count fingers at 3 feet.
- Bilateral Deafness (38 CFR 3.350(a)(5)): Absence of air and bone conduction in both ears—established when VA audiology shows bilateral hearing loss meeting the 100% schedular evaluation under 38 CFR 4.85 / 4.87.
- Complete Organic Aphonia (38 CFR 3.350(a)(6)): Organic speech-organ disability (DC 6519) constantly precluding speech communication.
- Breast Tissue Loss or Radiation (38 U.S.C. 1114(k); 38 CFR 3.350(a)): In a woman veteran, anatomical loss of 25% or more of tissue from one or both breasts (mastectomy/partial mastectomy under DC 7626), or radiation treatment of breast tissue.
Creative organ rules, erectile dysfunction (DC 7522), and stacking limits
The most common pathway to smc-k is service-connected erectile dysfunction (Diagnostic Code 7522, rated 0% under 38 CFR 4.115b with Footnote 1 directing review under 38 CFR 3.350(a)):
- One SMC-K award for “one or more creative organs”: A veteran with both erectile dysfunction (DC 7522) and testicular removal (DC 7524) or radical prostatectomy residuals for prostate cancer receives one creative-organ SMC-K award, not two.
- Post-service elective operations (38 CFR 3.350(a)(1)(iii)): Voluntary post-service elective operations (e.g., elective vasectomy) do not establish SMC-K unless required to correct an in-service surgical injury or advised on sound medical judgment to relieve a pathological condition or prevent future pathological consequences.
- Stacking up to three SMC-K awards and caps: Up to three SMC-K awards may be stacked for distinct losses. Combined with
10%–100%schedular pay orSMC-S, total monthly pay cannot exceed SMC-L (38 U.S.C. 1114(l)). Added to SMC-L through SMC-N or an intermediate SMC-P rate, total pay cannot exceed SMC-O (38 U.S.C. 1114(o)), excluding dependent allowances (38 U.S.C. 1115) and Aid and Attendance (38 U.S.C. 1114(r)).
Levels L, M, N, O, and P: Multiple limb losses, blindness, and step-ups
Under 38 U.S.C. 1114(l)–(p) and 38 CFR 3.350(b)–(f):
- Anatomical Loss vs. Loss of Use (
38 CFR 3.350(a)(2),(c)(2),(d)): Amputation is not required for loss of use of a hand or foot under SMC-K, SMC-L, or SMC-M; loss of use exists when remaining function is no better than an amputation stump with a suitable prosthesis (such as severe peripheral neuropathy or radiculopathy causing complete common peroneal nerve paralysis with footdrop and trophic/circulatory changes). For SMC-M and SMC-N, adjudicators assess whether amputation level, ankylosis, or paralysis prevents natural knee or elbow action with a prosthesis in place (3.350(c)(2)). Under 38 CFR 3.350(d), actual amputation is required for SMC-N except for bilateral loss of use of both arms (3.350(d)(1)) and bilateral blindness without light perception (3.350(d)(4)). - SMC-O and the Anti-Duplication Rule (
38 CFR 3.350(e)): SMC-O applies to bilateral shoulder-level arm amputations preventing prostheses (3.350(e)(1)(i)), paraplegia with bilateral lower-extremity paralysis and loss of anal/bladder sphincter control (3.350(e)(2)), specific combined deafness and blindness (3.350(e)(1)(iii)–(iv)), or conditions entitling the veteran to two or more rates in SMC-L through SMC-N with no condition counted twice (3.350(e)(1)(ii)). Under 38 CFR 3.350(e)(3), combinations require separate and distinct disabilities. - SMC-P Step-Ups (
38 CFR 3.350(f)): Under 38 U.S.C. 1114(p), veterans between SMC-L and SMC-N advance to intermediate rates (SMC L½, M½, N½) or the next higher rate up to SMC-O for asymmetric extremity or blindness/deafness pairings (3.350(f)(1)–(2)), loss or loss of use of three extremities (+1 step,3.350(f)(5)), an independent permanent disability or combination rated 50% or more (+½ step,3.350(f)(3)), or a single independent permanent disability rated 100% apart from TDIU (+1 full step,3.350(f)(4)).
Aid and Attendance (SMC-L, SMC-R1, SMC-R2) and TBI Care (SMC-T)
- Regular Aid and Attendance at SMC-L (
38 CFR 3.350(b)(3)and[38 CFR 3.352(a)](https://www.ecfr.gov/current/title-38/part-3/section-3.352)): Paid at SMC-L when service-connected disabilities leave a veteran unable to dress, bathe, adjust special prosthetics, feed themselves, attend to the wants of nature (including bladder voiding dysfunction or bowel care), or protect themselves from daily hazards. Under 38 CFR 3.352(a) and (c), only a regular need (not 24-hour care or every criterion) is required, and care may be provided by a relative. Under 38 CFR 3.350(b)(4), if a veteran is both permanently bedridden and in need of A&A, SMC-L is assigned on a bedridden basis (unless SMC-R applies) to avoid hospital reduction under 38 CFR 3.552. - Higher-Level Aid and Attendance (
SMC-R1andSMC-R2,38 U.S.C. 1114(r),38 CFR 3.350(h),3.352(b)): Veterans entitled to SMC-O, maximum SMC-P, or N½ plus SMC-K who need regular A&A while not hospitalized at government expense receive SMC-R1 (1114(r)(1)). They receive SMC-R2 (1114(r)(2)) in lieu of R-1 when they also require a daily “higher level of care” in the home—personal health-care services (physical therapy, injections, catheter care, or sterile dressings) provided daily by a licensed health-care professional or by an unlicensed caregiver (including a relative) under the regular monthly supervision (in person or by phone) of a licensed health-care professional—without which hospitalization, nursing home care, or other residential institutional care would be required. - SMC-T for Traumatic Brain Injury (
38 U.S.C. 1114(t),38 CFR 3.350(j)): A veteran not eligible for SMC-R2 qualifies for SMC-T (paid equal to SMC-O plus SMC-R2) when they need regular A&A under 38 CFR 3.352 for service-connected traumatic brain injury (TBI) residuals and, without such care, would require hospitalization, nursing home care, or other residential institutional care.
Housebound (SMC-S), C&P exams, VA Form 21-2680, and secondary claims
SMC-S (38 U.S.C. 1114(s), 38 CFR 3.350(i)) pays above 100% schedular compensation and below SMC-L when a veteran has a single service-connected disability rated 100% and meets one of two criteria:
- Statutory Housebound — “Total Plus 60%” (
38 CFR 3.350(i)(1)): Single 100% disability (schedular 100%, or TDIU based solely on one disability) plus separate, distinct service-connected disabilities independently ratable at 60% or more under 38 CFR 4.25, without home confinement. - Housebound in Fact (
38 CFR 3.350(i)(2)): Single 100% disability plus substantial confinement to the dwelling and immediate premises (or institutional ward) reasonably certain to continue throughout life.
Documenting SMC claims and accredited representation
- DBQs and VA Form 21-2680: C&P examiners complete DBQs from the VA Public DBQs portal (Reproductive Organ, Amputations, Foot, Peripheral Nerves, Eye, or Ear DBQs) to record loss of use, leg shortening (
3½ inches+), or blindness (5/200 or less). For A&A or Housebound in Fact, submit VA Form 21-2680 completed by a physician, PA, or APRN, plus VA Form 21-0779 if in a nursing home. - Secondary Conditions (
[38 CFR 3.310(a)](https://www.ecfr.gov/current/title-38/part-3/section-3.310)) and Combined Ratings ([38 CFR 4.25](https://www.ecfr.gov/current/title-38/part-4/section-4.25)): Secondary conditions—such as secondary erectile dysfunction from PTSD or diabetes, or diabetic peripheral neuropathy causing footdrop—may establish SMC-K or SMC-L, or combine to 60% for SMC-S or 50% for an SMC-P half-step bump. Check combinations with our VA combined rating calculator, estimate back pay with our VA back pay calculator, and note that under 38 CFR 3.552, A&A allowances are reduced on the first day of the second calendar month after government hospital admission and restored on discharge. - Accredited Help: Chartered Veterans Service Organizations (VSOs) (DAV, VFW, American Legion) assist free of charge, while VA-accredited claims agents and attorneys handle appeals. No representative can promise an outcome; verify credentials via the VA Office of General Counsel Accreditation Search.

Frequently asked questions
How does Special Monthly Compensation differ from standard VA disability pay?
Standard VA compensation under 38 U.S.C. 1114(a)–(j) pays 10% to 100% rates for average earning impairment. SMC under 38 U.S.C. 1114(k)–(t) and 38 CFR 3.350 compensates non-economic losses—loss or loss of use of organs or limbs, housebound status, and Aid and Attendance.
How does SMC-K work with my existing VA disability rating?
Under 38 U.S.C. 1114(k) and 38 CFR 3.350(a), SMC-K is a fixed monthly add-on paid on top of 0%–100% schedular pay, SMC-S, or SMC-L through SMC-N. Up to three SMC-K awards may be stacked for distinct losses, capped at SMC-L or SMC-O.
What is the difference between anatomical loss and loss of use under 38 CFR 3.350?
Anatomical loss means amputation or surgical removal. Under 38 CFR 3.350(a)(2), loss of use of a hand or foot exists when no effective function remains other than what a below-elbow or below-knee amputation stump with a suitable prosthesis would provide.
How do I qualify for Statutory Housebound (SMC-S) without being confined to my home?
Under 38 U.S.C. 1114(s)(1) and 38 CFR 3.350(i)(1), the 'total plus 60 percent' rule grants SMC-S when you have one single 100% service-connected disability plus separate, distinct service-connected disabilities independently ratable at 60% or more.
What are the criteria for VA Aid and Attendance under 38 CFR 3.352(a)?
Under 38 CFR 3.352(a), regular Aid and Attendance applies when service-connected disabilities leave a veteran unable to dress, bathe, feed themselves, attend to the wants of nature, adjust special prosthetics, or protect themselves from daily hazards. Only a regular need—not 24-hour care—is required.
Do I have to file a separate form to get Special Monthly Compensation?
The VA must consider SMC automatically when medical evidence or a rating schedule footnote—such as Footnote 1 to 38 CFR 4.115b for erectile dysfunction—shows entitlement. For Housebound or Aid and Attendance claims, submit VA Form 21-2680 completed by a clinician.
